Position Summary:

SUMMARY

Administers basic nursing care for residents in accordance with procedures and state/federal regulations. Completes appropriate records. Maintains safety and sanitation standards. Works under general direction of RN/LPN. Work is routinely reviewed for soundness of judgment, adequacy and accuracy. Scope of job duties are complex and the quality of work contributes to resident and co-worker safety and well-being. CNA must be proficient in basic nursing care to meet the resident’s physical and emotional needs. Is responsible for establishing and maintaining excellent customer service.

EDUCATION AND EXPERIENCE:

Must have a working knowledge of basic nursing skills as acquired through the satisfactory completion of a state approved training program. Must maintain current enrollment in the Commonwealth of Pennsylvania. Must maintain ongoing CEUs (must complete 12 hours per year). Access to PHI (Protected Health Information) will be based on Manager/Directors approval related to job responsibilities.

Dining Services Specialist

Position Summary:

SUMMARY:

Prepare/serve/deliver meals and/or set up and clean dishes, dining rooms and kitchen as assigned for the purpose of providing quality meals at established dietary/nutritional levels. Additionally, perform assigned duties of neighborhood specialist, cashier diet clerk and/or stock room clerk. Follow specific, detailed department instructions, policies and procedures, including established sanitation and infectious control standards. The scope of the job duties is of moderate complexity and the quality of work contributes to the nutritional/dietary well-being of the resident. Is responsible for establishing and maintaining excellent customer service.

EDUCATION, AND EXPERIENCE:

Experience in a health care facility preferred. Thorough knowledge of HACCP procedures and PA Dept. of Agriculture Sanitation Certification is helpful.

Personal Care Attendant

Position Summary:

SUMMARY
Provides physical and emotional support to residents who are alert, oriented and relatively independent in accordance with CKV-TBHC procedures and state/federal regulations. Assists residents with activities of daily living and general housekeeping. Documents observations and interventions in residents’ medical chart. Supervises self-administration of medications up completion of required training. Assists Personal Care Activity Coordinator. Works under the general direction of the Personal Care Administrator; receives clinical supervision from the LPN Charge Nurse. Independent judgment is frequently required.

EDUCATION AND EXPERIENCE
General use and application of high school education. Working knowledge of care and understanding of elderly as would be acquired through life experiences or a minimum of 6 months of related experience. Continuing education as required per state/federal regulations. Proficient in basic written and verbal communication skills. Scope of job duties requires routine interpersonal contact with residents, families and coworkers.

RN Supervisor

Position Summary:

SUMMARY:

Displays initiative and demonstrates independent judgment in prioritizing duties and responsibilities. Coordinates and supervises nursing care for shift; assists in the planning, implementation, and management of quality nursing care for Health care residents in accordance with state/federal regulations. Assists the Assistant Director of Nursing with projects as assigned. Participates in development and implementation of Nursing Department philosophy, goals and objectives. Scope of job duties are diverse and complex and the quality of decisions made contributes to the physical well-being and safety of the residents. The RN Supervisor must be proficient in and knowledgeable in clinical and management skills and in communication skills. Responsible for establishing and maintaining excellent customer service.

EDUCATION AND EXPERIENCE:

Satisfactory completion of a Professional Nursing Program and PA Registered Nurse licensing, plus a minimum of 5 years of professional clinical and progressive management experience. Licensed Staff Members are required to have an active CPR certification Maintains current CPR certification. Attends educational seminars/conferences related to duties and responsibilities.

RN/LPN

Position Summary:

SUMMARY:

Within the current and appropriate Nurse Practice Act, the RN is responsible for administration of resident’s medications following proper policies and procedures. Assess resident for condition changes, while documenting appropriately in the electronic computer program. Monitor dining activities to identify concerns or changes in resident’s needs. Supervises and monitors on the floor the delivery of care by Caregivers according to resident care plans, as well as any staffing conflicts or concerns that may arise. Respond to Caregivers request for assistance, correcting inappropriate or unsafe techniques. Provide input into education and improvement needs for the Caregivers. Provide exceptional customer service to residents, families, team members and outside vendors. Respond positively to delegation and direction given to them from the RN Coordinator/Supervisor or Nursing Administration.

EDUCATION AND EXPERIENCE:

Working knowledge in general nursing care acquired through a professional nurse program and licensure in Pennsylvania (RN or LPN). Licensed Staff Members are required to have an active CPR certification. One to three years of previous work experience preferred. Continuing education as required per Community standards and state/federal regulations and to enhance professional standing and growth. Access to PHI (Protected Health Information) will be based on Director’s approval related to job responsibilities.
