JOBS

Case Manager

Position Summary:

Hamilton Health Center is currently recruiting for a Case Manager to work under the guidelines of the Healthy Start program. This position assists with recruiting program participants and provides case management support services to preconception, prenatal and inter-conception women, infants, and children and their families. Educational qualifications include a bachelor’s degree in human services with a minimum of two years in human services work required. At least one years’ experience as a case worker preferred. Must have knowledge of community resources and programs and have respect for contributions and needs of multiple disciplines. Able to demonstrate accurate, clear, and concise verbal and written communication, documenting confidentiality regulations as required, possessing basic computer skills. Must have access to personal transportation and have a valid driver’s license if operating company vehicles or driving clientele. Bi-lingual Spanish speaking preferred.

Fatherhood Services Coordinator

Position Summary:

Hamilton Health Center is currently recruiting for a Fatherhood Services Coordinator for our Health Start Program. This position serve as lead contact for Healthy Start fatherhood services, providing outreach, case management, coordination and follow up of services, and education specific to physical, mental, and social wellness of fathers/father figures. Educational qualifications include a bachelor’s degree in human services with a minimum of two year of experience. Able to demonstrate accurate, clear, and concise verbal and written communication, documenting confidentiality regulations as required, possessing moderate computer skills. Must have access to personal transportation with a valid driver’s license if operating company vehicles or driving clientele. Bi-Lingual Spanish preferred but not required.

Medical Assistants

Position Summary:

Hamilton Health Center is currently recruiting experienced Medical Assistants to help support our growing Clinical Departments. This position provides assistance to Providers with direct patient care in an ambulatory care clinic setting under the supervision of a team leader, including initial patient triage, registration, and administrative services, assists with direct patient care, and performs specialized medical procedures of a routine nature, as dictated by established clinical protocol. Must be a graduate of accredited medical assistant program and be CPR Certified or able to obtain certification within 6 months of job placement. Qualified candidates must be able to demonstrate knowledge of health care services, including understanding of MA role and have excellent medical terminology knowledge and basic data entry computer skills. Bi-lingual, Spanish required.

Primary Care Providers – Physicians and Nurse Practitioners

Position Summary:

Board Certified or Board eligible required. Bi-lingual Spanish speaking preferred.

Registration, Customer Service Representatives

Position Summary:

Hamilton Health Center is currently recruiting experienced Registration, Customer Service Reps to help support our growing patient population. Our Call Center positions schedule patient appointments, manage the provider’s schedules in all departments according to set protocol of each department and direct all incoming calls of the PBX in a manner that will enhance corporate image and increase customer satisfaction. Our Registration Check-in/out positions register/check out patients according to department protocol to ensure the timely flow of patients in a manner that will enhance corporate image and increase customer and provider satisfaction.  High School graduate or GED required, associated degree or formal business/technical education preferred.  A minimum of one (1) year experience in customer service based role, medical/dental registration or other related fields required, medical terminology knowledge and basic data entry computer skills required. Bi-lingual, Spanish required.
